The day job uses the ADA4817. 400Mhz GBW for biological sensing. Audio gear at home uses the LF353, because it was easily available at Radio Shack & sounds good enough. The absolute best of the best for audio seems to be the OPA227.

For high-precision, low-frequency stuff I have a stock of MCP6V01's. +/-2 uV max voltage offset with +/-50nV/C drift (yes, it's a chopper). Noisier than some others, but relatively inexpensive for what you get.
Oh, and for audio you can't do much better than the NE5532, at least for the price.
