I had this same kind of thing happen to me once. It was a cheap capacitor from a Chinese brand and it exploded and shot the casing off like a bullet. The pressure relief vent didn't break!

Do you have a source for those cheap little 9v to 5v/3v3 pcbs? I have only found them on kits from amazon.
