Depending on your location you should consider working with the mySociety (https://en.wikipedia.org/wiki/MySociety#Street_fault_reporting) charity/opensource project who already have a FixMyStreet website/app that could be extended to include reporting accessibility violations. Oblig. https://xkcd.com/927/
