@Giulio pointed out to me that the component values in the PDF schematic file do not match those in the picture of the populated board. The schematic for this project is Cadence/OrCAD and the PCB is Protel DX and I no longer have running setups of either of these packages*, so the PDF and gerbers is all that I have of this project anymore. 

I edited the PDF schematic drawing to correct the component values, and to add the D3/R13 LED indication circuit on the +5V rail from the edge connector.  Manufacturer Part Numbers for non-obvious components are included in the schematic. 

* Yes, I really was that crazy - I preferred OrCAD schematic tool and Protel PCB layout tool, so I lived in this crazy split universe for many years. I've since become enlightened and switched to KiCAD...
