Any 12MHz quartz oscillator, suitable to clock ATTINY13. Something like http://sjk-crystal.com/4-2-3225-smd-oscillator/169879/ for example.
I only built it on debug board and used clock source from that board.

Yes, it works. I just build it on a breadboard and my Sixaxis controller started charging.
